What to Bring to Your Appointment
Bringing the right information allows for a more complete evaluation and helps avoid repeating steps you’ve already taken. If available, bring:
- MRI or imaging reports (or access to imaging)
- A list of prior treatments, including physical therapy, medications, or injections
- Any relevant medical records
- Your insurance information
If you don’t have everything, that’s okay. Our team will still guide the process based on what’s available.
What to Expect During Your Visit
